Medical Plan and Basic Medical Plan are High-Deductible Health Plans. These plans allow you to choose from a broad network of health care providers and manage your own expenses. A characteristic of an HDHP is that it may be combined with a Health Savings Account (HSA) to pay for qualified health care expenses with tax-free dollars.
Your eligibility for HMO coverage is determined primarily by your home ZIP code. Refer to the Annual Enrollment event on benefits.com to determine your plan eligibility and coverage options.
Applicable only if you elect medical coverage for a spouse or same-sex domestic partner who has other health care coverage available (for example, through his or her employer).
Applicable to employees and/or their spouses/SSDPs who have coverage in a sponsored medical plan and certify during enrollment that they are a “tobacco user and are unwilling to quit,” or certify that they are a “tobacco user and are willing to quit” but do not enroll in their Wellness program’s tobacco cessation program by the following March 31.
